The Clearline Vision plans powered by Avesis provide covered members with a complete vision examination, a pair of spectacle lenses, and a frame. If the member elects to receive contact lenses in place of spectacle lenses and frames, Clearline Vision will provide a generous allowance of $200.

Spectacle Lenses include: (Standard single vision, bifocal, trifocal or lenticular lenses). Lens options are discounted when using a participating provider by as much as 20% off of retail pricing. Participating providers will carry an assortment of frames within the allowance for each plan offered by Clearline Vision. Contact Lenses are in lieu of frames and spectacle lenses and are not subject to a co-pay. Co-pays may apply for vision exams and optical materials.

Giving members more choices is a continuing goal for Clearline Vision. In addition to our in-network coverage listed above, Clearline Vision provides reimbursement coverage for members who elect to go to non-participating providers.

The popularity of laser technology to correct certain vision conditions continues to grow. Clearline Vision provides its members with a LASIK allowance of $150. In addition, Clearline Vision’s national network of providers will discount their service up to 25% prior to applying the LASIK allowance.

Vision care benefits are typically available to all active employees as defined by your employer. Benefits are extended to include spouses and dependents (see your Benefits Manager for group dependent eligibility rules).

The Clearline Vision plans are designed to give each member a complete solution for vision correction. If the member chooses, add-ons can be purchased at a substantial savings of up to 20% off of retail pricing. Even after members have exhausted their covered benefits for the plan period, will still be able to take advantage of deep discounts on additional eyewear from participating providers.
